  • List of nutritional supplements or vitamins you are taking
  • List of previous operations
  • Personal history of health problems

  • Completed patient forms, including the completed Medical Records Release form to maintain your continuity of care. Forms Coming Soon!

FHCB accepts a broad range of payment and insurance plans. In addition, many of our patients qualify for free or low-cost health care services based upon their income and family size. To see if you qualify for free or low-cost services, use the tool on our Fees page.

No patient will be turned away based on an inability to pay.
